Make a small loop of ribbon, and then fold layers of ribbon back and forth, lengthening each layer until you have the desired thickness.

Staple the layers together in the center and then wrap the center with a coordinating ribbon or trim, securing the backing with tape or a small amount of hot glue.
For an upscale designer look, insert a stemmed flower (live or silk) in between the bow and center ribbon, or slip an ornament, bell, or other find onto the center ribbon before wrapping it with the flat bow.
