The two most common ways to introduce yourself are Watashi no namae wa __________ desu. (My name is __________.) or Watashi wa Sumisu __________ desu (I'm __________.).

Inquire about his or her name.
Use the more formal form because you're meeting the person for the first time:
O-namae wa nan desu ka. (What's your name?)
O-namae wa? (Your name?)

After they tell you their name you should express pleasure.
Common ways to express this are
Yoroshiku. (Nice to meet you.)
Yoroshiku onegai itashimasu. (It's a pleasure to make your acquaintance.
